  • use this form to inquire about a scoring concern (not reviewable) or other concern
• Dance Scoresheet Request
  • use this form if you need to request your scoresheets to be resent via email
• Dance Missing Awards
  • use this form if you did not receive the correct amount of awards (medals, participant bags, team acrylic)
